Steve was informed by Pete Wisdom about Nixtin Pharmaceutics and its head, Jacob Erskine, who was the grandson of Professor Abraham Erskine. Jacob Erskine's father was Tyler Paxton, one of Steve's fellow volunteers in the Super Soldier program. Apparently, Tyler and Esme, Professor Erskine's daughter, met at the professor's funeral. As Jacob Paxton, he turned down several job offers from S.H.I.E.L.D. and MI:13. Wisdom explained that Nixton had apparently uncovered the Super Soldier serum.
Steve went to Madripoor, where Erskine was, and infiltrated the Sovereign Hotel, where a party for international buyers was being held. Steve found that Erskine's wife looks like a woman he met in 1941. Attempting to follow, Steve shot two security guards to see if they had any elevator keys, but they didn't, meaning that he would have to take the stairs. But when he reached the V.I.P. gymnasium, he was attacked by several security guards augmented by the Super Soldier serum. However, the serum they were enhanced by was dangerously flawed, leading to irrationally violent behaviour and at least one collapsed in combat with a brain aneurysm. Steve defeated the guards and managed to find Jacob Erskine, who revealed to have taken on his grandfather's last name to call Steve's attention. Steve asked him if he was out for revenge because he got to become a Super Soldier and not his father. Erskine replied no, for he considered his father a hero for his intelligence and codecracking work in WWII. Steve further accused him of attempting to sell the Super Soldier formula to America's enemies, to which Erskine objected, stating that he was perfecting the formula to cure cancer and he wanted Steve's help so that he could finish it. Steve didn't believe him, citing the guards and his wife as proof. Erskine then claimed that formula was given to the guards without his permission, possibly by his head of security. Suddenly, Erskine was killed by an unknown attacker through a quick and silent bullet from afar.
In his last breath, Erskine begged for Steve to protect his wife Anita. Steve then escaped as Nextin security guards rushed towards the disturbance. He then went to the sniper's position, only to find a rifle connected to a laptop. He went on to interrogating several criminals of Madripoor, but he found no clue about the attacker's identity. Steve went to one of Wolverine's refuges and contacted Sharon Carter, asking her to investigate about Myron Smith, Nexton's security chief. As he continued his search, Steve thought of how Anita, Erskine's wife, resembled Cynthia Glass, a Nazi spy he fell in love with. Glass died disobeying her superiors and protecting Steve from the Nazis. Steve followed Anita and Smith and tracked them down to Nextin's summer retreat. He met Anita, who claimed to somehow know Steve. Scared, Anita ran and Steve followed her. While he followed Anita, Steve then received a call from Sharon, who said that all records about Smith are false. One year ago, Myron Smith did not exist, up until Erskine changed his name and got married. Steve found Anita and Smith alone in an office. Then, Anita and Smith ware revealed to be robots created by Machinesmith. Steve challenged Machinesmith to fight, but Machinesmith sealed him in the room and deactivated the Super Soldier serum within him.
In his childhood, young Steve Rogers was picked on by others bigger than him, seeing him as a weak, easy target. In the present, with the Super-Soldier Serum inside him deactivated, Steve is weak as he was in the 1940s and is now a prisoner of Machinesmith. He takes Steve to a lab, where he plans to extract a blood sample to recreate the perfected Super-Soldier Serum. Machinesmith explains that he is the one that convinced Jacob Erskine to change his surname and help them perfect his grandfather's serum. Or more accurately, his android wife Anita had convinced Erskine to change his name so as to draw Rogers out. Machinesmith reveals that he has based Anita's image off of a picture of Cynthia Glass, which Erskine's father kept as a memento. After that, one of Machinesmith's thugs knocks Steve out.
Steve awakens in a cell, where he is contacted by Machinesmith through a hologram. Machinesmith says that he doesn't need Steve anymore, and then, several guards appear to take him away. However, Steve breaks free of the guards and manages to defeat them through skill. He then returns to the lab and reactivates Anita, who reveals to him that they are in a secret lab inside the beach house. Anita accepts to help Steve and leads him to a room that contains a Vita-Ray projector. Anita exposes Steve to the Vita-Ray, reactivating the Super-Soldier Serum inside him. Then, they are attacked by guards, but Steve defeats them.
Steve and Anita take a car and drive away. Anita tells Steve that Machinesmith is holding a meeting with representatives of terrorist-supporting countries in a Nextin base with an airfield. Steve then goes to stop him and asks Anita if she is in his inside, and Anita says that she is.
Machinesmith begins the auction of the Super-Soldier formula.
Steve and Anita communicate with Hank McCoy/Beast to come up with a plan to stop Machinesmith. Beast has a plan that can counter Machinesmith's ability to transfer his mind into nearby electronic devices. The plan involves Anita. They sneak into the base and Steve goes to stop Machinesmith, but not before asking Anita why is she helping him. Anita replies that she doesn't know, maybe she was programmed to feel anger.
Steve attacks Machinesmith and slices his head off with his energy shield. However, Machinesmith transfer his mind into nearby robots. Steve destroys them, forcing Machinesmith to return to his head. Anita appears and Steve reveals that she destroyed every electronic system in the base. However, Machinesmith replies that there is an electronic device still active, Anita herself.
Transferring his mind into Anita, Machinesmith tries to attack Steve, but Steve reveals that Beast has altered Anita's operating system, making it so that wireless traffic goes in, but not out. Essential, Machinesmith is trapped within Anita and self-destructs, presumably killing Machinsmith. Steve takes Machinesmith's briefcase and leaves.
In the Secret Avengers HQ, Hank tells Rogers that the vials in the case weren't the Super-Solider formula; they couldn't even cure the common cold, much less create a supersoldier. Steve then asks why was the mission was about.
In Detroit, Michigan, Jacob Erskine is revealed to be still alive and working for Aloysius Thorndrake of the Shadow Council. Erskine says that they only needed a sample of Steve's blood for comparison but they had to sacrifice Machinesmith for a ruse. They see a young man on the street and Erskine says that the young man has the same story as Steve, his mother died two years ago and he is suffering through a terrible economy. Thorndrake says that if the Super-Soldier formula only works on Steve Rogers, then the Shadow Council will create an army of Steve Rogerses.
